    This guide will help you decide what medical or dental plan works best for you and your family. 
     
    Nooksack Valley offers Northwest Benefit Network as the only vision plan for all employees.  This plan covers all family members. New employees must complete an enrollment form for the 2016-2017 school year and submit it to the district office by September 09, 2016. Employees that have any changes, must fill out a new form and submit it to the district office by September 09, 2016.  Please do not mail the completed form to NBN as it states on the enrollment form.  Send it to Marsha McInnis in the District Office.

    Summaries of Benefits and Coverage for All Plans Offered 
     
    Medical and dental plan years begin November 1 of each school year.  New employees benefits begin October 1. 
     
    Should you need to make changes to your plan follow the links to print out a copy of the enrollment and/or changes form you need.

    Should you change from Regence to Kaiser Permanente please use this form to receive deductible credit. Click here
     
    Should you wish to waive medical insurance please complete the Waiver Form and submit it to the district office before September 09, 2016 for open enrollment. (Click Here for the Waiver Form.)  You must do a waiver form each year that you waive.

    If you are currently enrolled in any WEA Select Plan and do not wish to make any changes, you will automatically stay in your current plan.  If you are a new hire or wish to make changes, you will need to enroll using the online system or by calling the WEA Select Benefits Center at 1-855-668-5039.
